After filling out all the necessary forms, send the Joint Declaration Form PDF to your employer's EPFO field office along with the required documents. If your name has been entered wrongly in the records of the EPFO, you will have to submit a joint application with your employer. You will also need to provide supporting documents for the name change.
The following documents are accepted for amendment: Passport, Driving License, PAN Card, Aadhar Card, Voter ID Card, ESIC Identification Card, Food Card, Copy of Bank Passbook, Certificate issued by the Registrar of Birth or Death, any certificate of education, Certificate as proof of service in a central or state organization.
To change the name of the father or spouse: You have to send a joint request on behalf of yourself and the employer to the concerned EPFO department.
For Date of Birth Correction: There is an option to make this change in the form. However, you will need to provide proof of date of birth along with the application.
For Correction in Joining / Leaving Date: Employer to the concerned EPFO Department.
For Change of Address: The EPFO portal as of now does not allow a change of address through the portal. Changes can be made at the time of PF transfer or withdrawal. The only thing you need to provide is your address proof.
